MyChart Patient Portal System Terms & Conditions of Use

This website ("Site") and related services are provided by Enloe Medical Center to its patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully.

I. Statement of Participation

Enloe Medical Center provides the website MyChart ("MyChart") to allow Enloe Medical Center patients and authorized patient representatives to view healthcare information, communicate with the patient's healthcare providers and staff, request an appointment, and request prescription refills/renewals.

To access MyChart, the user must agree to be bound by the terms of this MyChart Agreement ("Agreement") for the material and data contained in MyChart. If you do not agree to the terms of this Agreement, please do not use or access this Site for any purpose.

II. Privacy & Security

Your privacy is of the utmost importance to us and we are committed to protecting the confidentiality of your medical information. Any personal information you submit or that is maintained on MyChart is subject to all state and federal laws governing the security and confidentiality of medical information. Firewalls, passwords, encryption and audit trails are further used to safeguard your information. We have taken steps to make MyChart secure against unauthorized access and use. Please review Enloe Medical Center's Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information.

III. MyChart Features

The health information provided through MyChart is not a substitute for the advice of your personal physician or other qualified health-care professional. Always seek the advice of your physician or other qualified health-care professional with any questions you may have regarding medical symptoms, tests results, or your medical condition.

If you think you have a medical or psychiatric emergency, call 911 or go to the nearest hospital emergency department.

A.   Electronic Messaging

MyChart should never be used for urgent matters.

Enloe Medical Center will make its best effort to provide a timely response to messages or other communications submitted through MyChart. In some cases, providers and staff may not be immediately available, so you should allow at least 2 business days for a response. If you require a quicker response, please call your healthcare provider.

If your provider is out of the office or unavailable, messages sent via MyChart may be accessed by or shared with staff that assists the provider with your medical care in order to facilitate a timely response to your request. Your confidential medical information on MyChart will be accessible only to appropriate Enloe Medical Center staff.

B.   E-mail Account Associated with MyChart & Secure Communications

Notification messages regarding information in MyChart may be sent to your e-mail address. These e-mail messages, however, will not contain any confidential medical information. Any person with access to your e-mail account will be able to see this notification. Please take this into consideration when providing an e-mail address.

All communications between you and Enloe Medical Center using MyChart occur over a secure connection. All of the confidential medical information available to you in MyChart is protected and securely maintained by Enloe Medical Center.

C.   Access Code

When you initially enroll in MyChart, you will be provided with an access code. The access code you receive will give you the opportunity to create a personal login name and a password. Once this access code has been used it will no longer be valid. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if they were you.

You are responsible for taking all reasonable steps to ensure that no unauthorized person has access to your MyChart password or account. It is your responsibility to (1) control the disclosure and use of your login ID and password; (2) authorize, monitor, and control access to and use of your MyChart account and password; and (3) promptly inform the Information Services department at Enloe Medical Center of any need to deactivate a password. Enloe Medical Center recommends that you change your password on a regular basis.

D.   MyChart Mobile Application

A MyChart Mobile Application is available for download for iOS and Android devices via the Apple Store of Goggle Play by clicking on the Enloe Medical Center icon.

The Enloe Medical Center MyChart Mobile Application is intended for use only on a mobile device that is running an unmodified manufacturer-approved operating system. Using the Enloe Medical Center MyChart Mobile Application on a device with a modified operating system may undermine security features that are intended to protect health information from unauthorized or unintended disclosure. You may compromise the privacy and security of your health information if you use the Enloe Medical Center MyChart Mobile Application on a mobile device that has been modified. Use of the Enloe Medical Center MyChart Mobile Application on a mobile device with a modified operating system is a material breach of these Terms and Conditions.

IV. Use by Minors

An individual must be age 18 or older to request a personal account. Minors, ages 0-11 may not establish a MyChart account. However parents or guardians of minor children under the age of 12 may request proxy access to the minor child's MyChart account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions Agreement. Minor children between the ages of 12 and 17 will not be eligible to enroll in MyChart. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 12 or older may not view without consent of the minor patient. Because of these requirements, we can only offer limited access to parents of minor patients in this age category. Thus, when a minor patient reaches age 12, access will be limited in MyChart until he/she reaches age 18.

Patients age 12-17 may use MyChart to:

  • update insurance and demographic information
  • update Advance Directives
  • make changes to preferred pharmacy
  • By law, teens are able to obtain some kinds of health services or information without notifying their parents or guardians. For additional information contact Enloe Medical Center's Health Information Management department at (530) 332-5518.

    V. Proxy Access to My Family's Record

    A patient may authorize other individuals to access the patient's MyChart account by completing a Proxy Authorization Form and submitting it to Enloe Medical Center's Health Information Management department.

    For patients under the age of 12, access will only be granted to individuals with parental rights or legal guardianship over the patient and only to the extent that the party requesting proxy access can demonstrated the legal right to the patient's medical information. For all other age groups, please refer to the Proxy Authorization Form for detailed information regarding the requirements for access to another person's medical information.

    If you have been granted access to another patient's MyChart information and your legal relationship with the patient changes, or you wish to terminate your authorization for proxy access, you must inform Enloe Medical Center immediately by calling the MyChart Customer Support line at (530) 332-3860 or 1-866-882-3566. Enloe Medical Center reserves the right to revoke access to a MyChart account at any time for any reason.

    VI. Information Does Not Constitute Medical Advice

    Enloe Medical Center assumes no responsibility for how you use the information you obtain from this Site. You should consult with a physician or other health care provider regarding your own condition and how this Site content may or may not apply to you.

    You may be accessing your own medical information via the MyChart application and may wish to communicate with one of your Enloe Medical Center about that or related information. If you have an emergency, or think you need to speak to someone urgently, please do not use the email communication options provided to you by MyChart. Neither MyChart nor any other application available through this Site is a substitute for appropriate and timely contact with your physician. This Site is not a substitute for consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.

    VII. Limitations on Use of MyChart

    You agree that you will not upload or transmit any communications or content of any type that may infringe upon, misappropriate, or violate the law or the rights of any party. You agree that the following actions shall constitute a material breach of these Terms and Conditions:

  • signing on as or pretending to be another person
  • transmitting messages or material that infringes on or violates the intellectual property rights of others or the privacy or publicity rights of others
  • transmitting material that is unlawful, obscene, defamatory, predatory of minors, threatening, harassing, abusive, slanderous, or hateful to any person or entity
  • using MyChart's interactive services in any way that is intended or is reasonably likely to harm the patient or others
  • intentionally distributing viruses or other harmful computer code
  • VIII. Termination of Access

    Enloe Medical Center expressly reserves the right, in its sole discretion, to permanently terminate a user's access to MyChart due to any act that would constitute a violation of these Terms and Conditions. If access is restricted for planned maintenance of the MyChart system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyChart without advance notice.

    IX. Ownership

    All pages within this Site and any material made available for download are the property of Enloe Medical Center and/or its subsidiaries and affiliates. The content on the Site is provided for Enloe Medical Center and its patients only. The words "Enloe Medical Center" and its associated corporate logo are the trademarks and service marks of Enloe Medical Center. All other trademarks, service marks, and logos used in this Site, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of Enloe Medical Center or the third party that may own the trademark, service mark, or logo.

    X. Accuracy and Integrity of Information

    Although Enloe Medical Center attempts to ensure the integrity of the Site, it makes no guarantees whatsoever as to the correctness or accuracy of the Site. It is possible that the Site could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy arises, please inform Enloe Medical Center so that it can be corrected. Information contained on the Site may be changed or updated without notice.

    XI. Links to Other Sites

    Enloe Medical Center makes no representations whatsoever about any other website that you may access through this site or any entity that may post its advertisements on this Site. When you access a non-Enloe Medical Center site, please be advised that it is independent from Enloe Medical Center, and that Enloe Medical Center has no control over the content on that web site. Enloe Medical Center does not endorse or accept any responsibility for the content or use of any other linked site. If you decide to access any third party sites linked to this Site, you do so entirely at your own risk.

    XII. Website Revisions

    We may revise the information on MyChart, or otherwise change or update MyChart, including these Terms and Conditions, without notice to you. Enloe Medical Center may also make improvements and/or changes in products and/or services described on MyChart or add new features at any time without notice. We encourage you to periodically read these Terms and Conditions to see if there have been any changes to our policies that may affect you. Your continued use of MyChart will signify your continued agreement to these Terms and Conditions as they may be revised.

    XIII. Website Security

    For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

    You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Enloe Medical Center by viewing the digital certificate by clicking on the lock icon in your browser.
